Privacy Statement
NTRODUCTION
Your privacy is very important to PDG Innovations LLC and its affiliates (‘we”, “us” and “our”). This Privacy Statement explains how we collect, protect, use and share your personal information (i.e. information that identifies, relates to, or could be reasonably linked, directly or indirectly, with a particular identified or identifiable natural person or household).
PDG Innovations LLC uses the App to hold, preserve and most importantly communicate patient dental implant data. Data can be entered once a patient has a surgery to place a dental implant in patients jaw(s). Data is entered by a dentist who already has an established Doctor/patient relationship with the patient who is having a dental implant placed or already had an implant placed. A dentist cannot enter the data or access the App without a current United States dental license and an existing NPI#. This will be verified by the App when the doctor signs up. Make sure the data held by the doctor matches the data on the national registry. Npiregistry.cms.hhs.gov
Data on patient’s dental implant will include all important and pertinent information that the patient can securely hold and easily access on the App. Data will include but not limited to; manufacturer of Implant, size of implant, Lot #, Reference # of any such parts associated with the placing and restoring implant, healing abutments, custom or temporary abutments, x-ray(s) of implant(s), photos of implant, bone grafting or biologics and anything the treating dentist deems important for the implant data for prolonged success and future communication to another dental provider that would be asked to maintain said implant. Data will be entered by manually entering the data on the appropriate dropdown screen or taking a photo on the appropriate screen of implant data. MAKE SURE THE PHOTO CONTAINS THE LOT # AND REFERENCE # from the package. The x-ray data will be entered by taking a photo of the x ray of the implant on a digital or computer screen.
An estimated 3 million dental implants are placed in the United States every year. Currently, there are over 20 different types of dental manufacturers. Each dental implant manufacturer has their own set of drivers and equipment to access and restore or maintain their own dental implant system. The goal of the PDG Innovations App is to help efficiently communicate the patients implant data to a current dentist or any future existing dentist(s). Because of the many numerous types and manufacturers of dental implants currently being placed by dentists it has become increasingly confusing and complicated if the patient is to move, relocate, go on holiday or vacation, change dentists, or have their current dentist retire or stop practicing. Currently, each new dentist can have a significant challenge trying to figure out the patient’s current implant(s) if they cannot easily find or get a hold of the patient’s previous dentist or provider. Sometimes dentists and patients are waiting days, weeks or longer to figure out what implant they have in their jaw(s). Furthermore, if the dental implant does fail for any reason and the patient has moved or changed practices from the dentist who placed the implant, both the Lot# and Reference # are required to receive a warranty on the failed implant. This data is currently difficult to find and/or maintain. PDG Innovations feels this to be unacceptable and not utilizing current technology to help the doctor/patient relationship. Not enough is being done to improve the patient’s investment into their implants. Patients would be in control of their data on their PDG Innovations App and would be in control of only releasing their data to a providing dentist of their consent.
INFORMATION WE COLLECT FROM YOU
For the treating dentists/doctors we will collect data based on establishing you as a real licensed dentist in the United States. Your information is not sold, shared or given to any other party and is used solely for the purposes of proving you as an existing licensed dentist. We then use this data to make you a registered user on the PDG Innovations App so you may enter dental implant data for patients when needed. We are only collecting personal information, including sensitive data, from you and will endeavor to only collect the information that is strictly necessary to fulfill these purposes. For the patients (former, potential and current) we collect name, email address, telephone number, date of birth, and current zip code. This information is not sold, shared or given to any third party and is only used to establish your identification as a patient user on the PDG Innovations App so you can always have your implant date securely stored and accessible. The patient will be in control of which treating dentists they share their implant data with. We are only collecting personal information, including sensitive data, from you and will endeavor to only collect the information that is strictly necessary to fulfill these purposes.
WHAT WE DO WITH THE INFORMATION WE COLLECT?
We use your personal information for several different reasons and purposes including:
To set up and otherwise manage your member account on the App
To gather demographic information about user trends, such as age, gender and types of implants being used. Your name will never be shared.
To provide certified dentists with information on actual patients who have dental implants or going to receive a dental implant in the future.
WITH WHOM DO WE SHARE YOUR PERSONAL INFORMATION WITH?
Your information could only be shared with dentists who you have a current doctor/patient relationship with. Any PDG Innovations App dentist is not allowed to use any of your persona information on the App for any purpose other than doing business with PDG Innovations LLC. In addition, any information we collect, including Personal Information, may be disclosed as part of any potential or actual purchase, reorganization, merger, acquisition, debt financing, sale of company assets, or similar transaction, as well as in the event of insolvency, bankruptcy or receivership, in which case Personal Information could be transferred to third parties as one of our business assets.
We may also disclose your Personal Information if we believe that doing so is required by law, court order or legal process; under the discovery process in litigation; to enforce our legal rights, policies or contracts; to collect amounts owed to PDG Innovations; when we believe disclosure is necessary or appropriate to prevent physical harm or financial loss or in connection with an investigation or prosecution of suspected or actual illegal activity; or if in good faith we believe that disclosure is otherwise necessary or advisable.
Users or patients of the App may also have their information shared between dentists that the patient has given prior consent. This is done to only improve the communication, accuracy and efficiency of any dental implant the patient has surgically put in and restored. This information will be immediately updated on the Analog App. In the dentists App and the patients.
SECURITY
The security of all Personal Information provided to PDG Innovations is important to us, and PDG Innovation endeavors to take reasonable steps designed to protect your Persona Information. Our App has security measures in place to try to protect against the destruction, loss, misuse and alteration, unauthorized disclosure or access of the information under our control. Unfortunately, no data transmission over the App can be guaranteed 100% secure. As a result, PDG Innovation strives to protect your Personal Information, we cannot ensure or warrant the security of any information you transmit to PDG Innovation, and you do so at your own risk.

UPDATES
We may update the Privacy Statement from time to time as we deem necessary in our sol discretion. When we do, we will revise the last updated date at the beginning of the Privacy Statement. Notice of such update and/or modification will be provided on our App or as otherwise required by applicable law. We encourage you to periodically review this statement to stay informed about how we collect, use and share any Personal Information. Your continued relationship with PDG Innovations after the posting or notice of any amended Privacy Statement shall constitute your agreement to be bound by any such changes. Any changes to this Privacy Statement take effect immediately after being posted or otherwise provided by PDG Innovations LLC.
